Physical Exam
PLEASE RECONFIRM YOUR PHYSICAL EXAM APPOINTMENT
Dr. Jones will need at least an hour of your time for your physical examination. If you don’t appear for your appointment, you will leave us with a large block of empty time on the schedule with no opportunity to refill it.
THEREFORE, PLEASE RECONFIRM your Physical Exam THE DAY BEFORE. If you do not call us by 5:00pm, WE MAY BE FORCED TO CANCEL YOUR APPOINTMENT.
Don’t Eat
Please don’t eat or drink anything (If you feel you must eat breakfast, please keep it “light” to allow more accurate lab results.) If your exam is in the afternoon, you may eat a light breakfast and maybe a light snack, but please don’t eat anything more substantial until after your exam.
Be Sure To Complete Your “Homework” Form
Prior to your annual exam, you should receive a “Patient History Worksheet”. Please take time to fill this out now, so you have plenty of time to think about any details you want to mention or questions you need to ask at your exam.
Be on Time – We’re Like Southwest Airlines
Forget any prior experiences you may have had with doctors’ offices…THIS OFFICE RUNS ON TIME. That means that if you arrive on time you can always count on being seen promptly.
If you were to arrive a few minutes late for a Southwest Airlines flight, you would expect the plane to have left without you. Our office operates the same way.
Just as we ask that you be on time, Dr. Jones will not allow the schedule to slip and make you late because some other patient before you didn’t arrive on time. This firm policy is for your benefit.
Please Don’t Be a No-show
If you realize you’re running late, please call us. We’ll try to accommodate you. If you’re going to have to miss your appointment altogether, please LET US KNOW ASAP so we can give that time slot to another patient.
